• Care Coordinators make it possible for seniors with physical or cognitive disabilities to remain at home and live as independently as possible.
• Care Coordinators assess needs, determine eligibility for programs and benefits, and secure and monitor services in the home and community.
• Southeast Senior Services has care coordination programs in Ketchikan and Juneau and the rural communities of Angoon, Hoonah, Metlakatla, and Yakutat.
