Home and Community Based Programs
Southeast Senior Services
offers a range of Home and
Community-Based (HCB) services designed to promote the health, safety and independence
of senior citizens:
- Senior Community Centers featuring meetings of senior citizen groups, advocacy
teleconferences, cultural and social activities, adult classes, informational
forums and resources
- Nutritious Meals at the Senior
Centers for persons age 60 and
older
- Health Promotion Activities including exercise classes, blood pressure checks, flu
shots, health fairs and support groups.
- Adult Day Program (The Bridge), providing a safe and therapeutic setting
for seniors with special needs
- Care Coordination to assess seniors' needs and connect them with
services
- In Home Services, including picking up mail, running errands,
shopping and housekeeping
- Care-a-Van Transportation for seniors and persons with
disabilities
- Gatekeeper
Program to identify isolated seniors
and connect them with critical HCB services
- In-Home Counseling for seniors and their family
members
- Meals on Wheels for seniors recovering from surgery or
illness
- Respite Services to give family members a break from
caregiving
- Senior Information and Caregiver Resource
